Athletic Requirements
Welcome to Gilmour Academy Athletics! There are several requirements of the student-athletes, as well as parents, prior to participation. Below is an outline of these items. Please refer to this page throughout the academic year and especially prior to each sports season to ensure all of the necessary requirements are completed in order to compete. Failure to complete these items means you will not be able to participate in any sports-related activity including, but not limited to, practices and games. These requirements are within the guidelines of the National Federation of State High School Associations (NFHS), the Ohio High School Athletic Association (OHSAA) and Gilmour Academy.

Where to Send Completed Forms
Drop off the completed forms to the Athletic Office located within the Athletic Center or mail the forms to:

Why Make Additional Copies of Completed Forms
It is important to have a copy of the completed required forms in the event of an injury or a change in medical history. In addition, our school office will need an additional copy of the Emergency Medical Authorization (EMA) form and a copy of the completed physical will be needed if you choose to participate in CYO, travel or other outside teams. Please make extra copies of all completed documents prior to sending to the Athletic Department.
Preseason Meeting
At the beginning of each sports season, parents and student-athletes are required to attend a meeting to be made aware of the obligations required in order to have the privilege of participating in interscholastic athletics as a representative of Gilmour Academy and the OHSAA. Attendees will review the school’s essential eligibility standards, athletic policies and procedures, as well as the code of conduct and individual team rules and requirements. At the conclusion of the meeting, parents and student-athletes are required to complete the eligibility checklist before the student-athletes can become eligible to compete for Gilmour Academy in a regular-season contest.
